WHEN SHOULD THE FORESKIN BE CUT?

The foreskin is the thin layer of skin that covers and protects the head of the penis (glans). In some cases, the foreskin may experience issues that require surgical circumcision:

  • Phimosis: This condition occurs when the foreskin is too tight and cannot be retracted from the glans. This can make hygiene difficult, leading to infections and pain during sexual intercourse.

  • Paraphimosis: This happens when the foreskin is pulled back but gets stuck behind the glans and cannot be returned to its original position. If not treated promptly, this condition can cause swelling, pain, and restrict blood flow.

  • Balanitis: Due to poor hygiene or phimosis, infections can occur, leading to pain, redness, swelling, and discharge.

  • Presence of genital warts or chronic ulcers: In some cases, lesions on the foreskin need to be removed to prevent the development of more serious conditions.

Indications for Circumcision

Circumcision is typically indicated in the following cases:

  • Phimosis causing difficulty in urination or sexual intercourse.
  • Paraphimosis that cannot be resolved by normal retraction.
  • Recurrent balanitis that does not respond to other treatments.
  • Prevention of sexually transmitted diseases (STDs), infections, or penile cancer in high-risk cases.
  • Additionally, reasons such as aesthetics, personal preference, religious beliefs, or cultural factors may lead men to choose circumcision.

Methods of Circumcision

  • Traditional Method (Surgical Cutting):
    • Procedure: The doctor uses a scalpel to cut the foreskin, then stitches it up with absorbable or non-absorbable sutures.
  • Stapler Method:
    • Procedure: An automatic stapler is used to cut and staple the foreskin, and the staples will fall out on their own after about 10-14 days.

Post-operative Care After Circumcision

  • Wound Care: Keep the wound clean, change dressings regularly, and use antibiotics as prescribed.
  • Pain Management: Use pain relievers as directed by the doctor.
  • Avoid Sexual Activity: Refrain from sexual intercourse and masturbation for at least 4-6 weeks to allow for complete healing.
  • Monitor for Complications: Contact the doctor immediately if there are signs of infection, excessive bleeding, or other issues after surgery.
